I understand the first use box is going to be the first time no matter how long ago it was, but when it comes to filling out the nature and frequency of use box, am I doing a timeline from the start, or in the past 7 years?
Take your entire period of use and average the frequency. Was it daily? weekly? biweekly? monthly? quarterly? Or break the periods up into blocks. (ie. 5 years of weekly usage and then 5 years of monthly usage. )
