This is where things may get a little squishy: I don’t think this means a person’s SF-86 has to be 100% squeaky clean, as I’ve heard of folks with some issues get an interim. So this must be a judgement call to some degree.

Correct! You can have minor issues that are not serious (disqualifing) and get an interim.
