I am not exactly sure what the confusion is, the question on the SF86 is pretty straightforward. Where it says in the last seven years have you had any account or credit card suspended charged off or canceled for failing to pay as agreed you should mark yes. Then when it says, provide the associated account number, put the account number. Describe the property involved, credit cards accounts. Provide the amount, $ however much it was. The reason for the issue, identity theft. Current status, charged off. Date resolved, whenever that was. ETC… Just follow the instructions. Each account should have its own listing.
