Medical Records Release

I am working through EQIP now for public trust. Does everyone have to fill out a medical records release or is it only triggered based upon how you answer specific questions?

I just want to give my doctor a heads up in the event she will be contacted, but I will be responding no to any mental health issue questions. Is the records release still required?

Best to fill it out on all. For a period I know they were not sending them up unless they answered yes to the counseling and treatment question.

Everyone fills out a general release and a medical release.

Be honest when answering all medical questions. The doctor isn’t always contacted. It depends on the situation.
