For my employment history, for this one internship that I had, my first supervisor got a new promotion around two months in, so I was told that I would be working under a new (2nd) supervisor for the remainder of my time there.

For the supervisor contact info section, should I list only my 2nd supervisor, my first one, or both?

It will only let you list one. Just list the most recent one, but be ready to provide the other info if asked. Or you could put it in the optional comment section.
